Insert > Form then insert the form fields (text box, text area, dropdown
box, radio button, check box etc) into your page.
Right-click the form and go to properties, insert your emai address in the
field marked "Email".
There are otherways to read submissions such as loading the info into a
database rather than sending it to your email, it can be sent to a text file
and email address - there are several combinations you can use.
