Can not get Front Page Form to Email us when submitted

I have a form that I created in FP that used to work until we updated the
form, and moved it from one web site to another via FP publishing.

All FP extension directories were created OK when published (we've published
several times to several Web servers, both Windows/IIS based and Unix
Based - all with extensions installed & configured)

The form is set up to e-mail results when it's submitted - however when we
save the form, we get the following FP error message:

This form is being created on a disk based web or the FrontPage server
extensions have not been configured to send e-mail ...etc...
Would you like to remove the e-mail recipient?
yes/no
-----------------------

Properties of the form are set correctly (same as previously) and as far as
we can tell, SMTP Services and FrontPage extensions are installed correctly
and set up for e-mail to indicate the correct SMTP Server and email
addressses to be used.

IN addition to the problem saving the form discussed above, whenever the
form page is accessed by a visitor (or through a browser locally) it shows a
line that says "[FrontPage Save Results Component]" at the top of the form,
and whenever the form is submitted, the user gets the following error
message:

FrontPage Run-Time Component Page
You have submitted a form or followed a link to a page that requires a web
server and the FrontPage Server Extensions to function properly.

This form or other FrontPage component will work correctly if you publish
this web to a web server that has the FrontPage Server Extensions installed.

Click the <Back> arrow to return to the previous page.

I have already done the following:
Re-created the form.
Switched web sites to several different platforms (XP, 2000, Unix/Linux)
Checked that FP Extensions and SMTP services are installed & as far as I
know, configured for e-mail
Read and followd KB Articles 197605 & 196489

Any ideas?

Check with current host/admin to see if the extensions have been configured
to process email.

Thanks, but I AM the host admin, and as the original post indicates, as far
as I can tell, the extensions HAVE been configured to process e-mail.

And you are opening the FP web using http mode
(not a disc based web as in C:\...)
Run a health check on the FP SE

Yes, I am opening the web in http: mode.
It is a web on an IIS server which I am accessing via browser (for example:
http://ourweb or http://www.ourweb.com
I have run a health check (IIS/Ourweb/properties/Check Server Extensions)

Do you have the SMTP server running?

Does your ISP allow you to send email when you are not logged in as a users
of their mail server? Most host do not allow this, since it would be easy
for spammers to access their mail servers.

Also there are no option under the SMTP Server or FP Extensions to add any
type of remote login info.

What happen when you publish the site to a web host that has FP extensions?
