I'm trying to modify the header to add check boxes but whenever I enter the
header my forms grey out. Any ideas?

Form fields in headers aren't supported in any version of Word.

The technique in http://gregmaxey.mvps.org/Add_Toggle_Objects.htm will let
you put boxes (that are characters in MacroButton fields, not form fields)
into headers.

There are some drawbacks to this:

- In order to click the boxes to change their state, you first have to open
the header area. You can do that by double-clicking in the header area, or
by clicking View > Header.

- If you're sending the form to other users to fill out, you have to create
the form as a template (.dot file), not as a document (.doc), because it
uses AutoText entries and those can't be stored in documents.

- The macros will cause Word to display an ugly macro-virus warning box when
you open the form, and you'll have to click the Enable button to make the
macros available. You can get around this requirement by digitally signing
the macro project.

I want the header of certain documents to include approval by an individual
in the organization. A check box by their name would alleviate having to
insert an e-signature which we need to avoid.

Right now as a workaround I am having the approving authority open the
header and insert the Wingdings checkmark as a symbol. It works, it's just
not ideal.

Place the form fields in a frame, which you can then position on top of the
header.
