How can I get Access 97 Chart to show specific number?

Hi im trying to make a database up using the classic access 97. The data
that will be entered will be manually typed from course questionares, each
trainee has 4 selections they rate between 1 and 5 (5 being highest)

Say 20 people are on the course I need the database to show me in a chart
how many rated for example course administration as 5 on course 87/06 etc.

I was trying to set up a form to do a simple filter/query to find this but
it never worked.
