How can I connect a survey to an order form?

My order form has a short survey that must be completed. How can I connect
them so that when the "submit" button is clicked it takes the view to the
order form? Is this via a hyperlink to that page? Is there a better/easier
way?

Have two forms, the survey and then post to the form...

I hate to be really stupid but I'm not sure what you mean. We are using
eMartCart shopping cart for the products. Is adding the survey form
something I can still do?

You would need to contact the author of the eMartCart to see if this would be a easy modification.
