Calling a form from another form's button press

Hi

I have a form that has a button that i would like to use to open another
form (the second form will contain a text box for free text input). When
the user types some text in the text box and presses the submit button, the
text should be able to be read into a oleDb querie.

My app has the main from, and a new form called inputs.cs.

Any guidance would be appreciated.
