Calendar Printing Assistant crashing...

I'm having a problem with the Calendar Printing Assistant in Outlook 2007
SP1. When I launch the CPA it loads properly but when I select a template to
load the CPA crashes. I have verified the "Calendar Printing Assistant for
Outlook 2007 Addin" has loaded at startup by opening Outlook - Tools - Trust
Center - Add-ins. I have unregistered and then registered the cpaoaddin.dll,
uninstalled and reinstalled Outlook, Service Pack 1, and the CPA client.
Still my problem remains.

Here is what I have found in the Appliation Event Log:
Event ID: 5000
Source: Microsoft Office 12
Description:
EventType clr20r3, P1 cpao.exe, P2 12.0.4518.1014, P3 45418247, P4 cpao, P5
1.0.0.0, P6 45418247, P7 52de, P8 1a4, P9 system.typeinitialization, P10 NIL.

Tried updating it to Service Pack 2 level already?
Also make sure that your Office 2007 installation is at Service pack 2
level.
