I have a user who has her reminder options set to play a sound, unfortunatly,
when a she gets a meeting reminder there is no sound played at all. ive gone
through and verified that she can hear sounds, and that this particular wav
file she is using is valid.

this is on an exchange environment using outlook 2007.

any ideas why it would not play sound on meeting notifications?

Did you check the reminder sound set on the actual appointment? Open the
meeting and look on the reminder dropdown - at the very bottom you can
choose a different sound than the default reminder wave.
