business contact record must have a name before it can be saved er

I cannot seem to publish (save) a modified business contact form in business
contact outlook. Each time I try to either run the form or save as I get an
error message saying the above and or that it does not recognize some of the
properties.

Business Contact Manager 2007, windows xp sp2, home system not on network,
shared data base, admin privileges, properties for business contact file
include use business contact form, show as an email address.

I am stumped......
