My list of likely suspects:
1) Something's wrong with the CLR (.NET 2.0). Try uninstalling .NET
and reinstalling it from the BCM disk.
2) Outlook and BCM versions are not the same. Perhaps Windows Update
upgraded Outlook to SP1?
