broken hyperlink - Form Action

note: I am running Frontpage 2003

I am creating a web page, and I have two broken links, I can't seem to
fix...they are in or part of the web page directed to the forms and requests
pages I created. The report says they are the type "Form Action" can anyone
give me a set of steps I should follow. I tried removing the hyperlink to the
form from any other page in the web site and got the same message, I tired
reinstating the hyperlink and got the same message, I tried looking at things
such as my privacy statment and the google link I have and well...with or
with out them linked I have issues. Please help.

note this is run off of a different server, if that makes a differance.
there are actually two places I experiance this problem and here are the urls

1. The first link below results in a 404.

2. http://wichlaz.com/tc/Confirmation.htm doesn't exist on this server.

3. The email address for receiving email, may not work since is not associated with your domain
name.
