Boot into safe mode

I got a customer Windows Vista Basic keep boot into Safe Mode even I
keep try to boot into normal mode. I had follow question:

1. How this happen for this Vista installation?

2. How do I fix this type of problem?

What procedure do you use to boot the computer?
What procedure do you use to shut down the computer? If you just unplug it
or use the power swtich to turn it off it will boot in safe mode the next
time?

Also, type MSCONFIG on the start-run line and hit enter,
click the Boot tab and make sure the Safe Boot is not checked,

This was a customer machine so I had no idea what happen. Now it
alway boot into safe mode even I force to normal mode.

can't even do that because customers forget admin password.

If you can get to the motherboard, remove the Bios chip to delete the admin
password,
Then reinstall it before rebooting.
