Basic Outlook forms advice

In Outlook 2002 I would like to create a form to send to other Outlook users
requesting them to review there current email details - if not correct -
provide them a simple format for the amended detail.

I do not have much experience of creating forms, the data is needed to
prepare for their Exchange server move.

Does anyone know of a sample I can download as a basis?
