automatic proxy doesn't work

I use an automatic proxy file:

I have certain addresses use the proxy server and others blocked

Sometimes the blocked address gets through and the allowed addresses get
blocked. Opera appears to be handling this fine. I even tried disabling the
proxycache for Internet Explorer - no change.
