Is there a way to have all incoming appointment requests marked 'private'
automatically?

It doesn't look like sensitivity is an option in rules but you should be
able to do it via a script.
Here is an example that accepts the request using rules-
http://www.slipstick.com/emo/2007/up071129.htm#3 - you'd need to change it
to set the sensitivity.
