I set up a Server Reply rule in outlook and it works great internally on our
network, if someone sends me an email, they get the auto reply fine. But if
I try from an external email address, I get the email from the external
address, but the auto reply never gets sent to them. Any ideas?

Exchange Server by chance? If so, be advised that both Automatic Replies
and Automatic Out of Office messages to the Internet are disabled by default
in Exchange Server; this is done to prevent e-mail loops (which can VERY
quickly bring an e-mail server to it's knees).
