On the Customer Billing Inquiry page, the Name and Email address are yellow.
On most of the other pages, which are in frames, I can't find the source to
look at it.

I don't know. I can't find the source for that page. Can you provide a
direct non-framed link to the form?

This is a guess.... I think Google toolbar looks for field names that it
recognizes, like "Name" or "Email" and then it can auto-populate those
fields. If the name you use for your field is obscure then it might not
recognize it.

The field names that I see on the "Customer Billing Inquiry" page are:
"Results from BILLING QUESTIONS Form on Web Site (www.mftinsurance.com)"
"InsuredName"
"InsuredEmail"
etc...

I would try simpler names.

I think you're right - according to
http://toolbar.google.com/autofill_help.html
Note to web page authors: You can ensure that AutoFill will work on your
pages by using field names defined in the ECML (Electronic Commerce Modeling
Language) standard, found at http://www.ietf.org/rfc/rfc3106.txt
