For the last month, every time I enter a new appointment, the first screen
that appears is one that only shows the Subject Line and the Location at the
top of the screen, rather than all the icons such as "Invite Attendees",
"Scheduling" "Save and Close", etc. I have to click on the tab for
"Appointment" in order to get those icons to appear, and then they cover the
Subject Line and Location.

Version of Outlook? Did you turn off the toolbars? (View, Toolbars menu)

I'm using Outlook 2007. When I'm entering a new appointment, there is no
option provided for toolbars, and I don't see an appropriate one in the
"Toolbars" option under Calendar.

Do you have the ribbon collapsed? Do you see Appointment, Insert, Format
text across the top? If so, they will pop out when you click the words or
you can expand them by double clicking on one of the words (i.e.,
Appointment or Insert).
