The Main Program Files menu in the Start dialog box has
suddenly become elongated to a single column. I would
like to know how to cascade (again) the listed Programs.
The associated program sub-menus should be in the
foreground.

Try this:

Control Panel > Appearance and Themes > Taskbar And Start Menu.
On the Start Menu tab click Customise then Advanced.
In the Start Menu Items section, scroll down to "Scroll Programs" and
uncheck it.

I also have this issue and did uncheck the scroll down feature. Now I have
two menus which is fine. Is there a way for it to create another 3rd
cascading menu? I got quite a bit and the second page is running off the
bottom.

And I did consolidate as much into groups as I could.
