AIW 9800 pro and reboot problem

I have just built the following and have been suffering random reboot
and screen going blank problems. I changed the card to an ATI 9250 and
so far all seem to be ok. I did not have these problems with my old
system based on ABIT NF7 board.

Any suggestions on the 9800 will be appreciated. I need the AIW
capabilities so what is the best X series option please considering I
need AGP? The Board only takes 1.5v graphics card

Asus A8V with 2 x 512mb Kingston memory
2 Hard drives - IDE
2 DVDs
1 front panel memory and USB
AMD 3700+ - not overclocked

Quality 350watt power supply or greater. Make sure each rail has sufficient
power.

Am using Qtec 650W. The readings are from :

Speedfan shows:

Temp2 : 34C
Vcore : 1.26V
+12V : 11.55V
3.3V : 3.36V
Vcc : 5.09V
Vin2 : 0.00V
5Vsb : 5.15V
Vbat : 0.00V

I cannot understand the readings but any help will be appreciated

The > +12V : 11.55V would worry me. Especially with your equipment and
problems. Especially if that's with the 9250. You might try disconnecting
unneeded drives for testing.
Power supplies is a regular topic in the pc.homebuilt newsgroup. A lot of
supplies don't put out their rated wattage. Even one that does may not put
out enough power on the rail(s) you need it on.
