Advance Buttons for Forms

I am developing an order form and would like to put in a button so that the
customer can add more lines to the form (example: 5 lines display for
quanitity and description - click here to add more lines). Does anyone know
how to do this?

If the field is a text-area more lines will be added automatically as
the user needs them.
Otherwise, please give a link to the page or post the form code here.
