Adding an 'upload' field to a FP Form

Hi,
I want to include a browse button and input field at the botom of a FP Form
so users can upload a file or compressed folder to be submitted with the form
and emailed with the results.

Does anyone know of a script etc that I can add to a FP Form to acheive this?

Sorry Guys,

I should have added that I have tried the upload feature [button] within the
FP Form list - but when I submit the form the form asked my for a username
and password?

It then submits but I do noy get any emailed results?

OK Guys - all fixed - no replies required.

I found where I was going wrong via the help section.

The FP form handler uploads the file to a folder on the web server (the
"_private" folder), it doesn't actually email it as an attachment.
