To answer the second half of your question...
Once you have added the title placeholders, change from viewing the slide
miniatures to viewing the outline. Right now, it is probably blank other
than a column of slide icons. Click after the first slide icon where you
need to add a title and type that slide's title. Arrow down, type the next
one, and so on. In my experience, it goes much faster than typing the titles
into the placeholders.
