Go to the section with the missing header. Then select File > Page Setup >
Layout tab – the problem is most likely that "Different first page" is
checked. If that is correct, uncheck the box and make sure that "This
section" is selected below "Apply to". Click OK.
If you insert a section break in a section that has "Different first page"
selected, the new section will inherit that setting.
