Adding a level to the TOC

I'd like to add a level (Heading 6) to my TOC and can't get it to appear.
I've tried:

Insert | Reference | Index and Tables
Click "Options"
Sscroll to Heading 6
Enter "5" in the entryfield (I'm not using Heading 5 and leave it blank)
Click OK
Click OK

When I F9 the TOC, the Heading 6 entries don't appear.

The options are expressed in the TOC field. Delete the current field and
enter a new one, or change the switches.
