Add a routing slip to a custom Outlook message form so it routes.

I have developed a custom Outlook 2003 message form which will be filled out
by a user and then sent through a number of approvers and then to the final
recipient. How do I add a routing slip to the Outlook form so it routes
automatically after each approver has entered their approval on the form?
