I want to create new item type (Ex : IPM.Note.testType) and create new
custom form for this type.
And i want to show this for on Outlook Web Access
Im working with exchange 2003

Outlook Web Access doesn't use Outlook custom forms. It has a completely
different extensibility model with documentation starting at
http://msdn.microsoft.com/exchange.

I looked in exchange 2003 sdk but i didn't find anything related to
this.
