I'll bet you have an Ositech modem installed. Search your Windows
directory for the file mdmgen.inf - then use that file when you click
on the "have disk" button. The standard modems will appear in the
list. I had the same problem and it took me several hours to figure
this out.

Subject: Add modem- manufacterers not listed on install wizard

When I use the Add hardware wizard to add a modem,
intending to use the "standard" modem driver, a list of
manufacterers to choose from does not appear! Why doesn't
Windows XP display the list of manufacterers to choose
from? How can I obtain the "standard" modem driver? My
hardware manufacterer says my hardware should work with
the standard driver.
Other info:
I upgraded Windows 2000 professional to XP professional.
