Select Tools, Forms, Design a Form. In that dialog select User Templates in
File System in the drop-down and navigate to your OFT file. When it opens
use the menus to Publish As and select the Personal Forms Library.
Exactly where in the menus Publish As is located depends on your Outlook
version, which you didn't specify.
