I have followed Frontpage's Help File. I have tried other online suggestions
and help, but when I try to add a custom hit counter, the image that appears
on the web site is [ Hit Counter] and not the custom image.

Are you http publishing to a remote/host server that has the FP server
extensions installed, which are required for the hit counter to work?

The FrontPage Server Extensions (FPSE) are not installed.
If your Host provides a control panel, you may be able to
install the FPSE yourself, or ask them to do so.

Alternatively, other hit counters are available that don't
require the FPSE. http://www.sitemeter.com See in use at
bottom of page http://www.marinachamber.com/

http://www.nedstatbasic.net See in use at bottom left of
page: http://www.theminutemanfoundation.org/

Most hosting companies have stats for your use.

You have to make the image(s) in your image editor. (however I assume you have
done this).

You need to create the numerals 0-9, save as the width x height the help file
says to do then in the "custom" field in the counter dialogue, browse to your
image file.

Upload the image file and page with the counter. The internal workings of the
program that makes the counter work will "slice" the image and display the
numerals as necessary per the number of hits you receive.

Also you need to publish to a server that supports frontpage extensions, and
specifically the hit counter function.

My server company does have the extensions. They have been turned on since I
first published. I have tried entering different addrresses for the image,
but none seem to work. I already have stats available through the hosting
company so this is not an issue. I only want a hit counter that matches my
site better. I've checked into other companies, but it seems like there is
always a catch to the counter. I want to remain comercial free.

If you hosting provider has installed the URLScan Tool, it can prevent
the Hit Counter from working.

I'll check with my provider. This may be the answer.
