If you hosting provider has installed the URLScan Tool, it can prevent
the Hit Counter from working.

Conversation: How to add a custom hit counter?
Subject: Re: How to add a custom hit counter?
My server company does have the extensions. They have been turned on
since I
first published. I have tried entering different addrresses for the
image,
but none seem to work. I already have stats available through the
hosting
company so this is not an issue. I only want a hit counter that matches
my
site better. I've checked into other companies, but it seems like there
is
always a catch to the counter. I want to remain comercial free.
