I have to ask how you are "uploading" the web.
If you are FTP'ing the site you will corrupt the extensions.
"Also" FrontPage will not publish all of the associated files
for the form when using ftp.
Make sure you publish using
http://yourweb.com and then
FrontPage will publish all of the related files.
I run into this issue when publishing to newly created
domains / accounts because I am forced to publish by ftp'ing
to the IP address. The instant I can publish to the domain
using http, everything works.
