2007 form or template that has un-editable text

Is there a way to create an outlook 2007 form or template that has
un-editable text and text entry fields or possibly Y/N check boxes or radio
buttons? The un-editable text would be one question per line requiring an
answer the idea would be that the recipient would tab from one field to the
next, respond to the question and send the e-mail back

You could implement the questions as captions for label controls and provide
text boxes bound to custom properties for the users to type in their
answers, BUT an Outlook custom form isn't going to be a practical solution
unless this is for internal use only and you can publish forms to the
Organizational Forms library on your company's Exchange Server.

E-mail would not be confined to an internal LAN or exchange server ( being
sent out to multiple blackberry external recipients ) Captions I don’t
believe would be helpful in that scenario.
