"Send updates only to added or deleted attendees" option is not available for
some clients running Outlook 2003 SP2 (11.6568.6568). If a meeting request
is updated to add an additional attendee, all attendees automatically receive
the update when selecting "Send Update" and the user is not prompted.

are you using any addins? I've seen it happen with a couple of addins.
