Just a guess, but check the start status for the Logical Disk Manager
Administrative Service. It should be set to Manual. If it is not, change it
to Manual.
Concerning installing Service Pack 4 in Safe Mode, you will likely have to
uninstall the IIS component of Windows, install the Service Pack, and then
reinstall IIS.
